Slab, insulation and dpm, what order?

I've now got from botom up 150mm hardcore, sand blinded, visqueen dpm, 100mm concrete (poured last Saturday so i take it its ok to cover now?) should it be another dpm on top of the concrete before the 125mm poly insulation or insulation then dpm??? The rest of the floor will be underfloor heating panels followed by 70mm sand/cement screed?
I've searched and its left me confused...

Insulation then dpm, but it doesn't need to be a proper dpm, a thinner sheet will suffice.
It's to prevent the water leaching out of the screed. I think there's a more technical term for it. Probably something like " de-watering".
