Sliding Aluminium Garden Door

This door no longer runs well. It squeaks and jams. The wheels under the sliding panel/door are ok but the problem is the wood underneath the alu base is rotted. The track is no longer flat in either plane. Anyone taken on a job like this?
I think I can remove the sliding door and fixed panel. Then unscrew the alumin'm base/runner.
Remove old wood. Cement the bricks if necessary. Put plastic damp membrane onto bricks. Rawlplug on the new wood. What am I missing? Will this work? Thanks in advance for any thoughts or advice. Richard

Ps. I've improved it by pressing in some slivers or teak as you may be able to see from the photo. That's not a long term fix though and its still not good.
