Speedfit/Pressure problem

Recently installed a full plumbing system using JG Speedfit.

On Saturday arrived at the site to find hot water pouring from the downstairs ceiling as the hot pipe had come off the shower and hot water was flooding through the ceiling. Thought this was just bad luck as the boiler had been installed for a week without any leaks.

While repairing the ceiling today the washing machine connection (col water) attached to the speedfit pipe using the insert and olive blew off and blew the olive clean off the pipe which is impressive as it would not go back over the insert.

Has anyone ever come across this before? Could it be just a build up of pressure causing the connections to blow off?

I know whats happend here, the standars speedfit inserts are too bulky at the end and when the pipe end is pushed into a compression fitting the olive is about 1mm from the end of the pipe causing it to blow off.

I would strongly recommed swapping all inserts in comp fittings to the smaller type, some hold for ages before they go.

Kutterlite fittings have a slightly deeper fitting depth than the cheaper ones.

Use the softer copper olives aswell, brass are not suitable for plastic pipe.

If running copper to JG pipe would you not just push the copper in to one end of the plastic fitting and JG pipe into the other end. (with the correct insert of course) ?
Or Are you saying that JG pipe can be pushed in to a compression fitting ?
Not being funny by asking this, I just didnt realise you could do that.

The JG Superseal pipe inserts have a bulky end, that does tend to compromise fitting into some compression fittings (Screwfix 69329).
Its best to use the ones without the 'O' ring - Screwfix 64318.
