threshold detail for new door opening in existing cavity wall

I am looking to make a new double door opening in an existing cavity wall, with a new threshold detail that will be level with the existing slab. The door manufacturer is offering a low profile metal 116mm wide threshold which will only just cover the cavity, & I am wondering if I should cut back the brick and block lower and cast some more concrete for the new threshold to sit on. How should I key this new piece of concrete into the existing slab?
I am assuming there is an existing dpm wrapping around the slab, should I wrap some more under & up the face of the new proposed concrete edge piece?
