Toilet seat fixture

Does anyone have any knowledge of how to remove this from a toilet seat. I’ve tried a lot of prising and twisting but it seems to be screwed in.

It was left by a previous owner and I have no idea how to remove it so I can fit a regular seat. The toilet is concealed so I cannot reach the underside.

I think it may be part of a disability seat.

Have you attempted to "Unscrew" the plastic disk at the base of the two holes? the one with the serrations around the inner surface of the upstanding lip?

Just a suggestion.
