Topping up system pressure

I've just topped up the pressure in our megaflow system 1/2 bar (up to 1 1/2 bar) after a new bathroom was fitted. But, after turning off the valve the pressure kept building right up to max. 4 bar. The valve was definately turned off and I've double checked this again. The boiler's release valve has now kicked and water is coming out of the overflow pipe outside. Anyone have any ideas as to why the pressure kept climbing?

Does the boiler have a disconnectable filling loop?

If so, did you disconnect it?

Because the valve that you think was closed wasn't actually closed.
 
Fill the system up to 1 1/2 bar and then remove the filling loop (if possible). If the system stops filling, then you know the valve is faulty. If the system continues to fill then water is obviously entering from somewhere else into the heating system. Have you had any new rads or towel rails fitted in your bathroom? Maybe one has been connected to a 'mains' supply pipe by mistake. I have seen this done.
