un-even concrete slab

hey guys, so i layed a concrete slab with the right amount of concrete, the ground was level but after drying for 5 days it became un-level. what is the correct proceedure of fixing this? Can i resurface even with the existing concrete?

did the ground move, e.g. where you had dug it out, or if it water under the surface?

how did you prepare the site, what did you put under the slab, what thickness?
