uPVC Door hinge tearing, door sagging

Hi! My front door is catching/sagging, and the hinge is tearing it seems. Would simply replacing the hinge fix he problem? If so, where can I get a hinge like this and is this doable as a DIY job?

Looks like an old Vitawin 3D flag hinge, easy DIY job if it's a like for like swap job

Thanks, when I went to get a new one I realised that they're adjustable. So I was able to make enough of an adjustment to fix the issue for now, nice one!
