What could have caused this?

"Don't know how many times I have to tell you this"

What?

It was aimed at the regulars. This problem arises weekly. When the grains in the plaster don't lie flat you get what is called 'suede' and it's hard to get rid of.

Guys, thanks for your help yesterday. Whether or not there was a fault with the plaster, on the walls I opted to use a longer pile medium mini roller which provided a significantly less patchy finish than on the ceiling. In due course I will re do the ceiling with another coat of white. Many thanks

Every coat will make it less patchy as the plaster gets covered with layers of paint.
