I found that the fuse described was indeed blown, and the reverse direction relay not switching reliably.     I contacted the manufacturer, Barber Wilsons. They have improved the design of this so that the spout is on a wall plate mounting instead of being supported by the supply pipes. Also, instead of screw in, it now pushes on to a spigot with o-rings and fixed with grub screw -- really nicely done...

I'll have 12.5 mm tile/adhesive + 12.5 mm board + 12 mm backnut, total 37 mm, which leaves no thread left to get the BSP...
