    Could I just swap these out for GU10 fittings which are double insulated and therefore if a lamp goes in future I don't have to remove the fitting but the lamp itself. Or could buy 230v double insulated led fittings without transformers.

    No, they are halogen ceiling burners, probably 50 watts each. Perhaps 35 if you are lucky. Equivalent LEDs are 3 or 4 watts. From the pictures they look like GU10s, which are twist to remove as advised above. Box of 50 here...

    The glass in the fitting protects you, the glass in the OP's hand (if Iggifer's correct) is the remnants of a defective GU10/MR16 lamp. They do very occasionally just fall to bits. Those fittings are almost impossible to do a lamp swap without (partly at least) removing them from the ceiling.

    Maintenance free JB

    The above are good for DL wiring, hide in ceiling, note position on site records and remove GU10 (or other similar fitting) to access. The shape makes them easy to pull out of 'the hole' for inspection or remedial work.
