I have quite a large (40mm) transition. The bathroom was insulated and underfloor heating fitted. What is the best way to cover this? My plan was to use something like picture #2, but I cant find anything that has a big enough size difference

Best way to resolve that are Stairrods Easy Shims to ramp the carpet up and then a standard or premier finish metal Z edge lipped over the tile.

It looks like you will need to plane the door unless a carpenter makes a wedge shape which butts up to the edge of the tiles. And if it does butt up, you will need a sympathetically coloured silicone to act as an expansion joint.
