BG 330+ (Glow-worm) boiler leaking condensate

We have an 20 month-old BG 330+ that recently started leaking water. BG were called out and identified a hole had melted in the plastic condensate trap. The heat exchanger & trap were replaced under warranty, but I am very concerned this may be a design flaw and could happen again. I understand it can represent a serious health risk.

Is anyone aware of similar incidents with this or related boilers?

Thanks - the problem is that the engineer carrying out the repair said he had not seen this happen before, but he thought that there may have been other similar instances of it elsewhere.

installers melt it when they're soldering the gas pipe connection. Seen about 3 or 4 like it on warranty calls on new build.

why was the heat exchanger replaced?
