bridge between wooden and concrete floor?

Hello
I need some advice please, i wish to lay porcelain tiles in my kitchen.
I have floor boards in one part and due to an extension i have a concrete floor in another.
So please what would be the best way to bridge the small gap(of about 3/4 of a centimeter) between the floorbaords and the concrete extension.
Looking at the previous posts it looks as though i will have to lay Exterior WBP Plywood over the floorboards and the concrete will need a bit of self-leveller but i don't know the best way to bridge the floorboards and the concrete floor.
So any help and/or advice would be appreciated :?:

read the sticky about how to level a very uneven wooden subfloor and use this method but flowing the levelling compound between the two subfloors.
