broken Drayton trv4 heads

Over the summer three heads have broken off these trv's. The ceramic inside has shattered, leaving the locking ring on the valve body. Am I right in thinking that they had been overtightened when fitted?
I've obtained replacement heads and would welcome some advice so this does not happen again.
