My house was built in 2001 by Countryside, but they didn't install any cavity insulation. Having removed a flue cover there is a void and not a scratch of insulation. I'm guessing there is no come back from Countryside.

The cavity is 45mm, so what's the best filling? Thanks in advance for any advice.

Seek the advice of a registered installer.

Perhaps your house did not need cavity insulation, so it was not installed.

There's quite a draft in the cavity, so I think insulation will help.
