vaillant combi boiler model no vcw gb 221 h

kept losing pressure,changed the releif valve which had had it but have now noticed the the pressure is high that the relief valve lifts and once the boiler is of the pressure is then low and we have to top the boiler water level/pressure up .
Whats the problem here
please need halp
 
Expansion vessel failed or needs recharging.
