Concealed toilet with an underwhelming flush

Hello all

I have a concealed toilet with a hidden cistern - the cistern fills to the top and empties on flush ... but never seems to do the job of flushing (especially toilet paper).

The cistern is a small one - but has always seemed to work ok ... but for the last 6 months it is struggling.

Again, it is emptying all water ...

Any ideas of the issue? The small outlets inside the pan itself? Something blocking down the waste pipe?

What would you check first .. as reluctant to replace given its all hidden behind plaster board etc.

If you live in a 'hard water' area of the country, it maybe the outlets in the bowl rim are a bit clogged with calcium.
You say it's emptying, but is it emptying slower than it used to?
Does the force of flush seem weaker?
 
It does seem weaker and yes, very hard water. How do I try and fix this?
