Concreting a floor

Hi

Have just taken up rotten wood flooring and have filled with hardcore with 4 inch to spare (more in places)n the area is about 5m x 3m

approx how many bags of concrete and how much (cubic m) or ballast will I need
also what is the best ratio for mixing
