DPM & Insulation on Concrete / Asphalt kitchen floor

hi, i have knocked through to open up the kitchen in my 1960's house and thus need to raise the level of the floor by 44mm. The old floor is asphalt and has broken up in places to reveal the concrete sub base. My plan is to raise the floor using concrete, screed to level it and then install laminate flooring.

Im guessing i should put a new DPM down (because the asphalt is damaged) as well as insulation but not sure whether to put the DPM straight onto the asphalt, concrete on the top of that, then insulation, then laminate flooring. Or concrete up to a level then put down DPM, insulation then laminate flooring (Hope this all makes sense!).

Also as its not a great raise in height (44mm) what sort of insulation would be the best to use?
