useally if the teeth are damaged etc.
check the motor could be just loose.

Everything seems OK with actuator in 'H' possition.

Switch off at mains and actuator goes to 'W'. When is it supposed to stay on 'M' ?

no it isn't it will fall to it's relaxed position with no power at all to valve.
which is hw (W)

it will only stay in (M) if both ch/hw are calling for heat.
and it will only stay in the (H) position after ch satisfied if you have power on the grey to hold it in last position.


remove the plastic top and watch it operate see where it clicks.
 
Actuator in awkward position in a cupboard can't see much but actuator is 'rocking' and ticking in 'M' position
