Electric underfloor

Thinking of installing electric underfloor heating (on top of floor - covered with self levelling compound).

Is this the order, working from lower to the finished floor :

- self levelling compound
- insulated mat
- electric floor / mat
- laminate underlay (still needed?)
- laminate

Questions:

1. What depth is the insulated mat and electric mat combined?
2. What electrics do I need in place to connect it up?
3. Would I still need a radiator?
