Extending concrete floor

Looking for some advice on how to extend a garage floor by a small distance (18cm) to accommodate new garage door frame. The existing floor is 2.3m wide but short short of 18cm to allow the bottom of the new steel frame to be bolted down.Current base appears to be about 10-12 cm thick. I need to know the best way to pour a concrete extension and ensure it stays attached to the existing floor. (Rods & Resin?) Also, what ratio of ballast, sand & cement and any sub base required. (bearing in mind I will need to drill into this once set to bolt the frame down. How would I lay any membrane to ensure it prevents damp ingress? Any advice gratefully accepted.
