The fireback generally sits flush, so as to be snug with the fireplace. But I can't think of a reason why it shouldn't sit back slightly. (No doubt there are plenty & we will shortly be advised)

Why should you want it to sit back?

It can be positioned wherever you please so long as you do not interfere with the flow of the flue.
However, it is important that you position the firebrick relative to any decorative elements fronting the fireplace.

A typical firebrick would sit so that the cheek edges finished about 20mm or so from the decorative frontage with the gap being filled with asbestos rope or pointed with fireclay.

Also. the closer to the front the fire the more you get heat wise.

That's what I did. The cast iron front sits into the recess so to make everything line up the fire back sits behind the opening
