Fixing this dripping kitchen tap

I have the following style kitchen tap and with the taps turned off the faucet is dripping and over time the drip has become more frequent. Is it possible to easily replace the cartridge or washer on these ? How do you remove it and how do I know what replacements to get?

Turn off the water .
Flick the chrome cap off the side of tap ,undo screw, pull handle off .
Undo cartridge with a spanner ,holing tap so you don't loosen it.
Take to plumbing merchants with handle .
Depending on how old they are ignore above and buy a new set as the central O ring will be the next to go.

Looks like the middle disc comes out, revealing a screw, remove cartridge using a spanner. Take to your local plumbers merchants.

Thank you all. I see that toolstation do a set of cartridges they all come in 1/4 turn but have different mm spline sizes. So I assume the only way to get correct size is take old ones out and take it to plumbers and get matching sizes?
