Tricky topic. I honestly think that the only fair way to do it is set testosterone levels for "male" and "female" categories and have regular compulsory tests.
But, that will destroy sport and turn people off.
Being a woman with more testosterone will be a fair advantage, but then, all other women should be allowed to supplement.
And a man with lower than average testosterone, should then be allowed to take drugs to raise it to alpha male levels.
Again, it would destroy sport.

The only real option is to have a clear rule in sport, no drugs are allowed that alter your testosterone levels at any time, before or during competition. Let the women with high levels be naturally gifted in sport, and don't question.

But it raises the next question - how do you determine if the level was natural, or and not artificially raised in the years before they started competing, giving them years of training advantage.

I’ve heard that many female Russian field athletes (shot putt, Javelin and discus) failed the basic urine test mainly because they couldn’t get their cocks in the bottle.

There are categories for those who are too small or not good enough in some sports. Boxing, rowing, judo all have weight categories. Weight lifting has competitions where your body weight is a factor as well, probably loads more. Edit: football with under 21s or sports with veterans categories.

For men who transition into women there are rules. I believe the main point in them at the moment is to cap testosterone levels. I don't think anyone is particularly pleased with them as rules, but they are reasonable and I haven't heard any better suggestions.

Competitive sport is slightly different from Opera, rubbish singers just won't get employed. But it's not ok for Pavarotti to take part in a childrens talent show and take first prize.
