Installing a new fireplace

I'm trying to install a new fireplace, new granite hearth and granite back with a wood surround, basically the previous hearth was concreted in place and I have got the hearth out and chipped the concrete away but the surface is not level, one side is higher than the other, what can I use to even out the hearth? Also what kind of strong adhesive can I use on the hearth and the back surround to make it stay in place on floorboards and brick.
