Hi,
I'm only a DIY'er, so am happy to defer to the pros!
But I believe when these have cracked on tightening (as they are supposed to), there isn't much you can do to stop a leak (apart from trying to tighten them a little more) - and it's a case of replacing the fitting.
When I have needed reducers, I have used the multipart ones, such as:
• The only compression fittings range manufactured in the UK• Create a mechanical heat free connection• WRAS Approved• Suitable for use on both gas and water• Suitable for Domestic and Commercial Heating and Plumbing Applications• 25 Year Warranty • Manufactured to EN1254

I've been happy with them, and there is space to add the PTFE/LS-X before you make the joint
