Is it possible to run a shower room extractor fan under the floor?

I have a bathroom in the middle of my flat (1st floor) and I need to install an extractor fan (there isn't one there today).

It's not possible to run the extractor fan across the ceiling but we can raise the room flooring (in the bathroom and the adjacent bedroom). The bedroom is approx 18ft long.

Is it possible to run an extractor fan in this manner?

Yes you can run an extractor fan duct under a suspended floor but ensure it slopes slightly to the outside so that condensation runs outwards. It will drip from the end of the duct so ensure it does drip where it will inconvenience your downstairs neighbour.

Does the room have any windows ? If not it should have had an extractor fitted. If rented then the landlord shopuld arrange for it to be installed.

A suspended floor with trap doors for access provides a lot of storage space

Even better, use an in-line fan and bolt it to the downstairs neighbour's ceiling.
