It’s in a cellar that’s has smells, no access from inside.

I would say that's an AAV, that's the spindle holding the flap visible in the centre. Access cap would be almost smooth underside, maybe with just a small nodule where the formed part has been cut off from the mould.
 
So vertical and above the spill line of the basin? What is fitted in the celler?

I think it was too low, definately lower than the basin waste and not much higher than the water level in the toilet.
