Following recent rainfall I have a patch on the inside wall I have included pics. The house is under 10 years old, is it a case of refitting the tiles above where the patch is? Some of the tiles look Ill fitting. Also the water comes down these tiles from a downpipe from the main top roof, would it be better to divert the water another way, Some look a bit Ill fitting thanks Rick

Internally how close does the stain get to the velux? The tile next to the velux looks suspicious with the interlock missing

the stain is from the wall outwards

Its difficult to tell where the water enters on vaulted ceilings as it has a tendency to run along the PIR in the roof.
I would reroute the main roof drain if possible.
Check out the tile next to the velux to see if the interlock is missing.
If you are lucky the tiles may not have been nailed and will be easy to remove to inspect underneath.
Do you have a phone with an angle finder? What pitch is the inside ceiling?

What does the verge look like?

The felt under tiles must be failing and have a hole. The water could be coming in above and running down felt until it reaches a hole.
Lifting tiles will show what the problem is.

Also the downpipe flashing looks raised? Never great with water coming out onto a roof like that.

Be worth getting it looked at and pictures taken along with a quote.
Other option is to go hire scaffolding or tower and have ago yourself if your that way inclined.
