Leaking Garden Tap

I've got a garden tap which leaks. A new washer stops the leak completely, but two or three months later it's piddling all over my patio again. Surely a standard washer (red in colour-fiber) should last longer than that--or is there another problem I'm missing??

Thanks for your replies.

The leak is at the point where the tap screws into a vertical cold water pipe and there is a fibre washer there (I've changed it twice--good for a couple months each time!). Yes, I have wrapped the threads with PTFE--but that can't last forever can it?
