leaking shower

I have just installed a new bar shower. The shower comprises of a metal pipe that leads up to a large shower head.The pipe connects to the bar via a deflector which has a button that sends the water to a microphone style shower head via a flexi hose. The connector from the deflector to the bar leaks slightly above the nut. The thread on the bar section has been wrapped with PTFE tape and the nut is tight. There is a rubber washer inside the pipe at the back of the nut. Anynhelp appreciated.
 
Don't do plumbing really; but has the washer been overtightened causing it to distort and thus leak?
