Leaking tap

hi ,
my shower/bath mixer is leaking out of the shower head or the tap depending on which ones on.
i have to turn off the service valve to stop the leak.

Any ideas why its leaking and how do i get this type of tap off?

i have attached photo, hope it is visible!!

how do i get to the washer?
 
You should be able to remove the tops from the hot/cold tap handles then you'll be able to locate the screw to remove the handle to gain access to the insert.

Pics to small.
 
I can't see the pic properly but yours looks like screw off going by the style of tap heads to ones i've done before.
 
hi,
tried to get to the washer but couldnt work out which bit un screws,
does the middle piece of the tap undo or do you turn the tap anti clockwise and keep turning til it comes off?
