Materials needed for a single storey extension floor

Can anyone confirm please what materials and quantities i would need to install a base for a single storey extension to be used as a utility room
Just want to make sure when i ring my local builders merchant that i ask for the right stuff !

Also, would it need any steel mesh in it ?

Dimensions would be approx 7.3m x 2.5m and as i say, its just a single storey

1 pint of milk and 6 tea bags.

And about 4 ton of mot1 (3.7 loose in the back of a small tipper)
1 ton of sand (or 1 bulk bag)
1 roll of dpm (or a sheet of the correct size +.5m each way)
3.7 m2 of ready mix concrete gen1 for subbase or c25/st2 for finish s3/4 slump( ;D )
