Part M - Sanitary requirements in a New Dwelling House

I'm currently trying to identify the sanitary requirements for a new dwelling house.

Would I be right in thinking that the only requirement is a Doc M pack in the bathroom?

I'm sure I read somewhere that the bathroom should be located on the primary entrance floor e.g. ground floor?

No a Doc M pack is for commercial use not in a dwelling, you (usually) just need an accessible toilet in the entrance storey and any old bathroom for any other storeys. Its all in here: http://www.planningportal.gov.uk/uploads/br/BR_PDF_AD_M_2013.pdf page 67. One hopes you are familiar with the rest of the regs if you are embarking upon a new dwelling project ....
