When tightening philmac mdpe fittings is hand tight enough or do water pump pillars need to be used?

You'll still need to use a wrench/pliers. Make sure it is nice and tight.

Yes, they do.

You'll get leaks quicker with philmacs if the pipe is scored.
Polyplumb is a better system with the hammer in inserts even if the pipe is badly damaged. And especially where you are joining unto old pipework.
