Ply or Fibreboard?

Prime neat with a floor primer then use a smoothing compound like level flex on uneven bits.
I'd ply with 6mm then do the screed.
Plywood with 12mm and then use less screed if it's still needed.

Just to throw this into the mix, what about sheet 6mm mdf to cover the existing floorboards? Its cheaper than ply and still a solid material
 
If you are just going to board over it for level, then it does not matter to much if it is plywood/hardboard/mdf.

There are slight differences, but at that thickness for this job it is rather much of a muchness.

6mm is a Okish, *good* plywood would be a bit stronger than MDF but 6mm is 6mm (i.e not much) and there might still be a little bit of flex over bumps.

Suck it and see, I would be more comftable with 9mm.
