Question - Only 'warm' water coming from shower/bath???

Hi all - DIY noob here. This is my current situation:
- Have combi boiler (no tank), all is working fine
- Hot water from kitchen tap is very hot
- Hot water from shower/bath is 'warm' (apporx 70% hotness)
- The shower and bath water tap is separate from the temp control mixer
- I have a separate mixer (thing that controls hot/cold). This just controls temperature
- Even if I have the mixer on max, the hot water is only 'warm'

Questions:

- What do you think the problem is?
- How easy is it to replace the mixer?
- Do I have to turn the mains water OFF before taking the old mixer apart?
- Could the mixer dial be 'off' by a notch or two that i can take the facia off and replace it making the 'max' level a notch or two more than what i have at the mo?

any help will be much appreciated

Hi, yes it has always been like this since we moved in 6 years ago so we just lived with it and boiled lots of kettle and added hot water in the bath. Anyway, the boiler is a Gloworm Swift Flow and the mixer is a AXOR (?) and they must be at least 6 years old each as they were there when we moved in...

is the pipe to the mixer only warm or hot ?
not sure cause I can't get access to it. as it's a very small flat, I doubt i'm losing heat in such short distance. I just know that water is very hot by the kitchen sink and only warm in bath/shower tap...do mixers have it's own thermostat?
