Radiator issue

So I have one radiator that only gets slightly warm at the top. It's always been like this since I moved into this house.

The flow pipe is hot but return is cold.

It's in an extension and was tapped off an existing rad in next room

I'm ok with diy so I've tried the following some things a few times, without luck.

Both valves each side replaced and confirmed on and work.

Radiator removed and cleaned out with a hose pipe a bit of muck but nothing much came out.

Entire heating system has been drained and refilled about twice.

No air when bleeding, done on all rads.

All other rads work ok

Could the rad be duff ? What's the chance ? As I hosed it out and water comes out other side.

Close off all the rads that heat up by their TRV ( or wheel head valve if they don't have TRV). See if cold rad now heats up.
