Replacing an old Ash/quarry tile floor

I'm planning on replacing an unlevel, damp quarry tiled floor with concrete but don't know how deep to dig out. Could anyone give advice on how deep to dig and what should go down, i.e hardcore, sand, dpm, insulation etc.
