Rip up concrete and put down grass !!

Back garden is wall to wall concrete floor ! approx area I want to grass is 14ft by 12ft ! the rest i will slab and gravel
If I rip up concrete in this area ! wot layers do i need to put down before i grass !
Is it top soil then put down grass !!?
 
an absolute minimum of 100mm of a decent topsoil or preferably sandy loam, assuming you have removed all the crap and are back to soil
