rising boiler pressure

I have a Ferroli 25c combi boiler. It normally starts at 1.5 bar and rises to 2 bar when running the central heating. Yesterday the pressure has risen to over 3 bar (the prv has not kicked in) and stayed there. The filling loop is definitely closed. What could cause this please?

because I have quite a large system a previous engineer already added an additional expansion vessel to the system.

Wouldn't this kick in or could both have failed at the same time?
 
Unlikely but possible. Could be the plate heat exchanger.
