Dear Experts,

Should a plastic gland like the one in the photo be waterproof? I.e. rainproof when used outside?

Should the soft washer be fitted inside or outside the enclosure?

Waterproof 'ish'. I find water always finds a way, especially over time.
Hence I put these glands on bottom/lower surface of enclosure (where possible).
And put loops on cable to cause water to drip off cable before it gets to gland.

I also put a small 2mm hole on lowers point of box / enclosure so that and water or condensation can escape.
(and I know that this feels wrong when done on a box that is says it is IP65 rated).

I always fit the soft washer on the outside.
 
IF the washer is on the inside then the water it is supposed to hold back has already got in there.
It goes outside.

Why the questions - do you have a leak?
Likely culprit is the washer inside the gland that surrounds the cable.
Often seen glands used that are too big for the cable, and so a tight connection is not made.
 
There was more corrosion inside than I expected when I opened it to change a battery. I think the problem is the washer. The cable is also a bit thin; I may put some tape round it.
