What sleeve & insulation for 25mm MDPE?

What do people use to sleeve & insulate a new 25mm MDPE supply - this one is coming up through a concrete slab just inside the exterior wall.

I think you can buy a ready curved pipe about 75 mm diameter.

Presumably you should use the water regs insulation.

The sleeve should be protected against entry to rodents.
