But what do we test to with regards PIR's?

Not so obvious on a PIR.

How many people try to establish the route and depth of buried cables, and if they have earthed mechanical protection or not?

Depth should be simple enough, have a look at where the cable enters the backbox. If it's less than 50mm from the surface.....
Earthed mechanical protection? Maybe not so obvious but you may get an idea by looking at the entry to the backboxes, if not then a code 3.

Everything I've read points to all concealed cabling, in or out of 'safe' zones.

Not yet, but according to Geoff Cronshaw

"For example, this means (in a
domestic installation) where insulated
and sheathed cables are concealed in a
wall at a depth of less than 50mm and
have no mechanical protection, they
need to be installed within the safe
zones and need to be protected by a
30mA RCD."
