Blocked heat exchange plate

No solution to your problem but in the hundreds of plates i have had out changed/cleaned etc i have never seen anything that resembles how gritty that looks . In one side of the plate let along both sides . Very strange

Have there been roadworks?

This very much looks like gravel in the fresh water. You say the cold taps are clear, but have you checked any filters in e.g. electric showers or (some) mixer taps? If you fill the bath with cold water and wait, do you find any ‘sand” in the bottom?

Do you have a loft tank?

Your water company might be able to test it for you.

No loft tank, all the taps and shower are clear I’ve never seen anything in them, I even checked the filter in my kitchen tap and nothing in there so it can’t be contaminated water I don’t think
