CH pipes in concrete floors

Can anyone point me to chapter and verse on what has to be done, according to the latest regs?? It has to be accessible and insulated, which as far as I've been able to glean means a tray with a ply lid, and 22mm insulation on either 15 or 22mm pipe.
 
What about under-floor heating, one unbroken length of plastic pipe in screed.

I've also seen plastic pipe in flexible conduit.

Funny you should ask cos I have a job coming up in a ground floor flat with a concrete floor. They want to put radiators in the bedrooms which will mean running pipework across a door way. Either I keep it open vented and run the pipes in the concrete floor, or if the boiler permitts, convert to a sealed system and run the pipe work at ceiling level.
