Concrete driveway repairs

I have a concrete driveway, the bottom end of which has cracked and has some fairly large holes. I don't want to fork out for a new driveway as I want to move, but what is a quick way of repairing it? For instance, how far should it be cut back and what's the best way of doing it? To what depth should I dig it out? What mix of concrete should I use to replace it? Any ideas? Would rather learn from other people's experience before I make a complete pig's ear of it.

Surface repairs of concrete are rarely successful so you will need to remove the full depth of damaged material, usually about 100-150mm.

This can then be replaced with more concrete mixed at a ratio of six ballast to one cement plus water.
