Concrete slab reinforcing

Any points on the following plan for my house extension slab

150mm MOT
30mm sand
DPM
100 MM. concrete
75mm insulation
60mm sceed
Flooring

I'm adding some a193 mesh over a soil pipe, see thread http://www.diynot.com/diy/threads/underground-soil-support.475849
but other than that should I reinforce the slab? (6m x 4.5m).

How do I get a 10?

When should you use some mesh and when not? I added one to my last extension by request of the BCO but I've not seen mention of it in the regs. That was a standard ground bearing slab with no drains underneath.
